We work closely with Supports Coordinators, families, and teams to ensure smooth, timely placements and ongoing support. We're currently accepting referrals.

Our intake process
How a referral becomes a placement.
Referral received
You email referrals@ with basic info on the individual and their support needs.
Initial review
We review the ISP and service needs to determine if we're the right fit.
Team discussion
Leadership reviews capacity, home match, and staffing alignment.
Meet & greet
If appropriate, we meet the individual, family, and SC to assess mutual fit.
Transition planning
Structured move-in plan with the individual, family, and SC involved every step.
What we ask from SCs
Context we need to do this well.
The more complete the referral, the faster we can respond thoughtfully. When reaching out, please include:
• Current ISP (most recent version)
• Behavioral support plan, if applicable
• Medical summary and medication list
• Family contacts and involvement preferences
• Timeline / urgency of placement
• Any specific home characteristics needed (ground floor, specific room setup, proximity, etc.)
