Notice of nondiscrimination
Integrity Human Services LLC complies with applicable federal and Pennsylvania civil rights laws. Admissions, the provision of services, and referrals are made without regard to race (including hair type, hair texture, or hair style), color, religious creed (including all aspects of religious observance and practice, as well as belief), disability, ancestry, national origin (including limited English proficiency), age (40 and over), or sex (including pregnancy status, childbirth status, breastfeeding status, sex assigned at birth, gender identity or expression, affectional or sexual orientation, and differences in sex development), and without retaliation.
Free communication and accessibility assistance
When needed to provide equal access, Integrity Human Services provides the following services free of charge and in a timely manner:
- Reasonable modifications for individuals with disabilities.
- Appropriate auxiliary aids and services, including qualified interpreters and information in accessible formats such as large print, braille, audio, or accessible electronic formats.
- Language assistance for individuals with limited English proficiency, including qualified oral interpreters and translated written or electronic information when appropriate.

Section 1557 Coordinator & grievance process
If you believe Integrity Human Services failed to provide these services or discriminated against you, you may file a civil rights grievance with our Section 1557 Coordinator / Compliance Office. Email support@integrityhumanservices.com with the subject “Civil Rights Grievance,” or call (888) 973-2652 and ask for the Section 1557 Coordinator. Assistance with filing is available on request.
Grievances are reviewed promptly and equitably. Information is kept confidential to the extent permitted by law and necessary to investigate the concern. Retaliation for raising a civil rights concern is prohibited.
External civil rights complaints
You do not have to use Integrity Human Services’ internal grievance process before contacting an outside civil rights agency.
U.S. Department of Health and Human Services, Office for Civil Rights (OCR): file through the OCR complaint portal, email OCRComplaint@hhs.gov, call 1-800-368-1019 (TDD 1-800-537-7697), or write to Centralized Case Management Operations, U.S. Department of Health and Human Services, 200 Independence Avenue SW, Room 509F, HHH Building, Washington, DC 20201. Complaints generally should be filed within 180 days of the incident, subject to OCR extension rules.
Pennsylvania Department of Human Services, Bureau of Equal Opportunity: call (717) 787-1127 or email RA-PWBEOAO@pa.gov. DHS states that complaints should be filed within 90 days of the incident.
Pennsylvania Human Relations Commission (PHRC): call (717) 787-4410 (TTY (717) 787-7279) or use the PHRC public-accommodation complaint process. PHRC generally requires complaints within 180 days of the incident.
